César (Eduardo Noriega) is a rich womanizer until one night he falls madly in love with beautiful, warmhearted Sofía (Penélope Cruz) -- only to become horribly disfigured in a car accident with his unstable ex (Najwa Nimri) the next morning. Doctors restore César's appearance, but he starts having nightmarish hallucinations that lead him to kill Sofía.
